You may print the following credit application form and mail or fax it to Blue Moon Trading Co. The order form is in rich text format (rtf) and can be opened with a standard word processor. You may choose to open it and then select PRINT from the FILE menu of your browser, or to dowlnoad/save it to disk and open the file in your word processor. )
